かかりつけ薬剤師は、毎回同じ薬剤師が継続してあなたを担当します。これにより、薬の飲み合わせや副作用のチェックはもちろん、生活習慣や体調の変化までをトータルにサポートできます。

- 複数のお薬手帳を1つにおまとめ頂けます

当店ではEmail・電話・LINE公式を用いたアフターフォローをおこなっております。かかりつけ薬剤師を指名いただいたお客様は、処方箋の有無にかかわらずいつでもお薬のご相談が可能です。

- 薬局店舗にて、かかりつけ薬剤師を希望する旨をお伝えください
- 薬剤師がご説明いたします
- 申込書類に署名していただき手続きは完了です(所要時間5分 – 10分)
専用フォームもしくはEmail / LINE公式でのお申込
- 専用フォームもしくはEmailもしくはLINE公式にて、かかりつけ薬剤師を希望する旨をお伝えください
- ご入力のEmailアドレスに申込書類をご送信いたします
- 申込書類を熟読・確認した後に署名していただき、署名済み書類を添付してEmailで返信、もしくは薬局住所に署名済み書類をご郵送ください
ご利用金額
3割負担の場合: 処方せん調剤をご利用の際に、通常50-90円程を追加でご負担いただきます。

What is a “Kakaritsuke Pharmacist”?
Your “personal” pharmacist is your “medicine partner” whom you can feel free to consult with in such cases.
A pharmacist who is close to your condition and your life.
A “kakaritsuke Pharmacist” or family pharmacist is the same pharmacist who is continuously in charge of you every time.This allows us to provide total support, not only for checking medication combinations and side effects, but also for changes in lifestyle and physical condition.

We provide follow-up services using email, phone, and our official LINE account. Customers who appoint a family pharmacist can consult with us about their medications at any time, with or without a prescription.

Application
In-store application
- Please tell us at the pharmacy store that you would like to appoint a family pharmacist.
- The pharmacist will explain.
- Sign the application form to complete the process (takes 5 – 10 minutes).
Application form or Email / LINE Official
- Please let us know that you would like to appoint a family pharmacist by filling out the dedicated form or sending us an email or even sending a message to our official LINE account.
- We will send you the application form to your email address.
- After carefully reading and reviewing the application form, please sign it and return it with the signed documents to our email address or mail the signed documents to the pharmacy address.
Fee
For those who pay 30% of the total cost with your medical insurance: An additional charge of about 50-90 yen is usually added to the cost of prescription dispensing.
