薬剤師がご自宅や施設に訪問し、
お薬の受け渡しから服薬サポート、体調チェック、医師との連携まで行います。

単に“薬を届ける”だけではありません。
その方の生活のペースや体調の変化に合わせて、安心して薬を使い続けられる環境づくりをお手伝いします。

たとえば、薬がテーブルの上に散らばっていたり、
前回お渡しした薬がそのまま残っていたり。
薬の袋に日付を書いていたけれど、順番通りに飲めていなかったり。

薬局のカウンター越しでは見えない「生活の中のヒント」が、訪問ではたくさん見えてきます。

そして、そのヒントをもとに

• 在宅療養中で、医師や看護師の訪問を受けている方

必要なのは、「薬局に来られない理由」があることです。
費用や制度についても、薬局でしっかりご説明しますので、まずはお気軽にご相談ください。

ご利用の流れ

1. 処方内容に基づき調剤したお薬をお届けいたします。
2. お薬の内容や服薬回数や方法をご説明いたします。その際、患者様の服薬状況や健康状況をお伺いさせていただきます。必要に応じて、お薬のセットやバイタルチェック等を行うことも可能です。また患者様の状況に応じて、主治医へお薬の種類や服用に関して提案をさせていただくこともございます。
3. 訪問後は主治医やケアマネージャーの方にご報告をいたします。

1. 専用フォーム・Email・LINE公式・お電話もしくは店舗にて、当店による在宅訪問を受けたい旨をお伝えください。
2. 主治医やケアマネージャーにすでにご相談されている場合は、主治医より当店に訪問開始の指示があります。そうで無い場合には、当店より主治医にご連絡・相談をさせていただきます。
3. 手続きが完了次第、在宅訪問を開始いたします。

それ以外の場合:

 

1. 主治医に「当薬局の在宅訪問を受けたい」とご相談ください。
2. 主治医より当店に訪問開始の指示がございます。当店にて必要なお手続きを行います。

A pharmacist visits your home or facility and
provides services ranging from delivery of medications to medication support, physical condition checks, and coordination with your doctor.

We do more than just deliver medicines.
We help create an environment in which people can continue to use their medicines with peace of mind, in accordance with their pace of life and changes in their physical condition.

the medicines were left untouched from the last time they were given to you.

• Those who have difficulty going out and going to the pharmacy
• Those who have difficulty managing their medications on their own
• Those who are concerned about forgetting to take their medications or taking more than one medication
• Elderly people who live apart from their families

• Those who are receiving home care and are visited by a doctor or nurse

All you need to have is a ” reason why you cannot come to the pharmacy“.

We will explain the cost and the system thoroughly at the pharmacy.

Process

 

1. We will deliver the medication dispensed based on the prescription details.

2. We will explain the contents of the medication and the frequency and method of taking the medication.　At that time, we will ask about the patient’s medication and health status.　If necessary, we can set the medication and check the patient’s vitals.　Depending on the patient’s situation, we may also make suggestions to the attending physician regarding the type and dosage of medications.

3.　After the visit, we will report to your doctor or care manager.
