Pharmacists’ Home Visits Deliver More Than Just Medicines
“Recently, it has become difficult for me to go out.” “I sometimes lose track of how much medication I have taken. “I worry about whether my parents, who live alone, are taking their medication properly…”
We have heard many such comments.
We want you to know that you have options to choose from when it becomes difficult to pick up your medications at the pharmacy ─
That is the ” pharmacist’s home visit.
Professional support at home for your medications
A pharmacist visits your home or facility and provides services ranging from delivery of medications to medication support, physical condition checks, and coordination with your doctor.
We do more than just deliver medicines. We help create an environment in which people can continue to use their medicines with peace of mind, in accordance with their pace of life and changes in their physical condition.
“We realized that because of the visit.”
For example, medicines were scattered all over the table, or the medicines were left untouched from the last time they were given to you.
The date was written on the medicine bag, but the patient did not take the medicines in the correct order.
During the visit, we can see many “hints in daily life” that we cannot see over the counter at the pharmacy.
And based on the tips
✔ How to take medicines ✔ How to organize them ✔ Information sharing with doctors
✔ Proposing drug treatment that is tailored to the person’s unique lifestyle through
As a reassuring partner for your family
A home visit can be a great relief not only for the patient but also for the family.
✔ Know the status of your medication even if you live apart from your family ✔ Reduce the risk of medication errors and side effects ✔ Smooth cooperation between medical and nursing care
By intervening between doctors, nurses, care managers, and family members, pharmacists can facilitate cooperation and support patients as one team.
Eligible applicants include
Those who have difficulty going out and going to the pharmacy
Those who have difficulty managing their medications on their own
Those who are concerned about forgetting to take their medications or taking more than one medication
Elderly people who live apart from their families
Those who are receiving home care and are visited by a doctor or nurse
All you need to have is a ” reason why you cannot come to the pharmacy“.
We will explain the cost and the system thoroughly at the pharmacy.
Process
1. We will deliver the medication dispensed based on the prescription details.
2. We will explain the contents of the medication and the frequency and method of taking the medication. At that time, we will ask about the patient’s medication and health status. If necessary, we can set the medication and check the patient’s vitals. Depending on the patient’s situation, we may also make suggestions to the attending physician regarding the type and dosage of medications.
3. After the visit, we will report to your doctor or care manager.
