ご利用の流れ
1. 処方内容に基づき調剤したお薬をお届けいたします。


2. お薬の内容や服薬回数や方法をご説明いたします。その際、患者様の服薬状況や健康状況をお伺いさせていただきます。必要に応じて、お薬のセットやバイタルチェック等を行うことも可能です。また患者様の状況に応じて、主治医へお薬の種類や服用に関して提案をさせていただくこともございます。


3.訪問後は主治医やケアマネージャーの方にご報告をいたします。

1.専用フォーム・Email・LINE公式・お電話もしくは店舗にて、当店による在宅訪問を受けたい旨をお伝えください。
2.主治医やケアマネージャーにすでにご相談されている場合は、主治医より当店に訪問開始の指示があります。そうで無い場合には、当店より主治医にご連絡・相談をさせていただきます。
3.手続きが完了次第、在宅訪問を開始いたします。
それ以外の場合:
1.主治医に「当薬局の在宅訪問を受けたい」とご相談ください。
2.主治医より当店に訪問開始の指示がございます。当店にて必要なお手続きを行います。

Community Collaboration

In cooperation with medical and nursing professionals involved with patients, we contribute to team medicine and nursing care by maintaining close communication and information sharing, as well as attending service manager meetings and community care meetings, with the aim of realizing optimal treatment, recuperation, nursing care, and living for patients and their families.
Delivery of Goods
〜We can also deliver daily necessities along with medications.
In addition to medications, the following items can be delivered together during a visit (*please order in advance):
- Hygiene products such as masks and disinfectants
- Health foods and nutritional supplements
- Over-the-counter medicines and antigen test kits
- Cosmetics, household goods, etc.
*Depending on the number and size of items, we may offer a separate delivery service.
Process
1. We will deliver the medication dispensed based on the prescription.
2. We will explain the contents, frequency and method of taking the medication.
