First time visitors will be asked to fill out a medical questionnaire. You may submit it in-store or online in advance. If you prefer to submit the form online, please use this form.
Please submit prescriptions issued by hospitals and clinics, your medication notebook (okusuri techo), and your health insurance card or “my number card” to which your health insurance card is linked at the reception desk.
If an electronic prescription has been issued,
(1)If you receive a prescription with your “my number card”: You do not need to submit a copy of your prescription.
(2) If you receive a prescription with a certificate of eligibility or health insurance card: Please submit a copy of the prescription.
Prescriptions are valid for 4 days after they are issued; if more than 5 days have passed, please ask the hospital/clinic where you were treated to reissue the prescription.
*If you would like us to issue a receipt in a foreign language, please ask our staff or contact us by e-mail or LINE. We will issue a non-English receipt at a later date. Please let us know the e-mail address, address, etc. of the recipient.
If you send us your prescription or a copy of the prescription in advance via our official LINE account, you can receive your medication without having to wait. Please see LINE Fast Track for details.
*If you would like to consult with a pharmacist regardless of whether you have a prescription or not, please use our “The Family Pharmacist ” service. In addition to being able to consult with a pharmacist anytime via official LINE, email, or phone, it is also possible to consolidate prescription drugs prescribed at multiple clinics and hospitals into a single medication book(okusuri techo).
Customers who visit our store can take advantage of our free aftercare service. If you have any questions about your medication or prescription, you can ask our pharmacists directly by phone, fax, email, or the official LINE chat function.
If you are a foreign language speaker, please use the official email or LINE account. Bilingual staff will respond to questions in English. For other foreign language speakers, we will use a high-precision translation tool as well as CAT tools to answer the questions.
On-site staff are Japanese speakers only. If you feel that you do not understand the medication, the prescription, or the explanation given by our staff, please use our aftercare service.
